Case Manager - Justice Diversion Program

Summary/Characteristics of Position:

The Case Manager Enhanced Offender Diversion (EOD)- is a professional case management position responsible for managing caseloads and identifying appropriate resources to facilitate participant success including. but not limited to, mental health and substance use disorder recovery, employment, education, life skills and housing.

Essential Functions:

  • Coordinates admissions referred by court referral programs.
  • Complete admission and orientation documentation.
  • Develop plans and creative solutions to ensure successful completion of program.
  • Monitor and document participant progress and compliance with treatment plans.
  • Work with Judicial Officers, court personnel, prosecutors, defense attorneys, and staff from other social, health, education and related agencies to meet participant needs and ensure compliance.
  • Prepare progress and court reports reflecting level of progress towards program objectives.
  • Attend court hearings and present status reports.
  • Ensure smooth transitions to the next level of care (warm hand-offs), is completed, to avoid gaps in service, and respond rapidly for needed support and/or intervention.
  • Facilitate appropriate connection and linkage with peer support specialists, education, employment, housing and/or other needed wraparound services.
  • May facilitate criminogenic thinking psychosocial groups on rotation with other case managers.
  • May transports clients and/or materials to medical, legal, recreational and other community appointments that support their recovery.
  • Attend interdisciplinary treatment team meetings and clinical staffing sessions related to client compliance and progress.
  • Perform related work, as requested.

Pre-requisite Requirements:

  • Bachelor’s Degree in a human service related field
  • One (1) year of behavioral health or criminal justice experience for non-related degrees.
  • Must possess valid Florida Drivers' License
  • Must be able to fully participate in Welle (crisis intervention) and CPR/First Aid training
  • Must pass a criminal history background check and/or fingerprinting

Reports to: This position reports directly to the Program Director.

Supervisory Responsibilities: This position does not supervise others.

Physical Requirements: Safety/Health Considerations:

  • Physically able to drive a vehicle for prolonged periods of time
  • Full participation in transportation-related training activities, including on-the-road driving
  • Ability to perform considerable reaching, stooping, and bending.
  • Ability to lift, push, pull and carry

Work Environment: This job divides time between a professional office environment, ACTS service locations, and county courthouse. This role routinely uses standard office equipment and mobile devices.

Travel: Ability to drive to various locations daily using a company owned vehicle. This position does not require overnight travel.
