Case Manager - Mobile MAT Program

Summary/Characteristics of Position:

The Case Manager position provides assessment for mental health and substance use issues, basic integrated and supported individual, family and group education, advocacy, linkage, coordination, problem-solving and monitoring of clients to optimize the functioning of individuals with substance disorders or co-occurring substance and mental disorders and promoting stabilization, rehabilitation and recovery.

Core Responsibilities/Essential Functions:

  • Completes admission and orientation documentation; may administer or interpret screening/assessment instruments.
  • Develops plans, which include services to accomplish habilitation or rehabilitation
  • Provides clients with a comprehensive listing of community resources, including social, legal and educational to support their needs.
  • Document all clients' contacts, care and response to support their treatment needs
  • Attends interdisciplinary treatment team meetings to discuss client's care and progress 
  • Engage clients to talk about their mental illness and substance misuse and encourage their participation in recovery steps 
  • Conducts life skills group sessions to promote self-sufficient, independent living in the community
  • May transport clients and/or materials to medical, legal, recreational, and other community appointments that support their recovery.
  • Attends multidisciplinary team meetings to review case status
  • Attends court hearings serving as a liaison for program, courts and person's served for reporting progress related to treatment goals.
  • Assists clients with obtaining county, state and federal benefits (social security benefits, vocational rehabilitation, transportation, etc.)
  • Assists with obtaining birth certificates, social security cards, identification cards, bank accounts and other needs
  • Maintains updated rosters and submit census data as required.
  • Completes program  documentation including but not limited to behavioral progress reports, incident/injury reports and grievances
  • Ensures smooth transitions to the next level of care (warm hand-offs) to avoid gaps in service and respond rapidly for needed support and/or intervention.
  • Provides follow-up services for up to one year after discharge from the program.
  • Performs related work as required

Pre-requisite Requirements:

  • Bachelor's Degree. (Human Services may be required for some positions.) 
  • Must possess valid Florida Driver License
  • Must be able to fully participate in Welle (crisis intervention) and CPR/First Aid training
  • Must pass a Level II DCF, AHCA and/or DJJ background check

Preferred Attributes:

  • Bachelor's in a human services related field
  • One (1) year of case management in a related experience.
  • Experience working with in a behavioral health setting

Required skills/Abilities

Reports to: This position reports directly to the Program Supervisor/Director.  

Supervisory Responsibilities: This position does not supervise others.

Physical Requirements: Safety/Health Considerations:  

  • Physically able to drive a vehicle for prolonged periods of time
  • Ability to perform considerable reaching, stooping, and bending.
  • Ability to lift, push, pull and carry
  • Full participation in transportation-related training activities, including on-the-road driving
  • Full participation in Welle (crisis intervention) and CPR/First Aid training.

Work Environment: This job divides time between a professional office environment, ACTS service locations, and various community resource settings. This role routinely uses standard office equipment and mobile devices.

Travel: Ability to drive to various locations daily using a company owned vehicle. This position does not require overnight travel.
