The Care Coordinator - Hospital Bridge is a case management position that functions out of medical settings, including emergency rooms, to provide the coordination and linkage of services for individuals recommended for behavioral health services- including substance abuse, mental health and/or housing needs.

The Intake Coordinator is responsible to greet visitors and individuals seeking care and provides basic education and initial orientation to ACTS services. The Intake Coordinator assists those served with intake/orientation documents as needed, and coordinates service payment options/collections. Provides brief, motivational interventions and outreach to maintain client engagement in service activities.
